Referrals

Dr. Posen is accepting referrals for:

• General Pediatrics (including for Spanish-speaking patients)

• Infectious diseases consultations, including post-travel medical concerns

• Newcomers to Canada​

• Travel medicine (pre-travel consultations do not require a referral, are not covered by OHIP, and will incur a cost - please see the Travel Medicine page for more details)​

You may refer to us by filling out one of the web forms above (not available on the mobile version of this website), or faxing a standard form to 416-491-0795. Faxed referrals must include the following information:

• Service you are referring to

• Whether the referral is Urgent or Not Urgent

• Patient full name and date of birth

• OHIP or IFH number - please include a copy of the Refugee Claimant Document if you have it on file

• Verified home address

• Verified phone number(s)

• Verified e-mail address(es)

• Referring physician name, address, telephone and fax numbers, and billing number

• For Newcomer to Canada consultations, please include a copy of the vaccine record and prior investigations, if possible
